How IRMAA works

IRMAA (Income-Related Monthly Adjustment Amount) is a surcharge that higher-income Medicare beneficiaries pay on top of the standard Part B and Part D premiums. Social Security looks at your tax return from two years earlier, so your 2026 premium is set by your 2024 MAGI.

The cliff problem

IRMAA is not phased in. If your MAGI crosses a threshold by $1, your surcharge jumps to the next tier for the entire year — often more than $1,000 per person. That is why this calculator also shows how close you are to the next bracket.

Can you reduce or appeal it?

If you had a life-changing event — retirement/work stoppage, marriage, divorce, death of a spouse, or a large income drop — you can file Form SSA-44 to ask Social Security to base IRMAA on more recent income. Planning moves (Roth conversions timing, capital-gains timing, QCDs) can also keep MAGI under a threshold.

What counts in MAGI for IRMAA?

Your AGI (Form 1040, line 11) plus tax-exempt interest (line 2a). It includes things like capital gains, IRA/401(k) withdrawals, Roth conversions, and Social Security included in AGI.

Why is 2026 based on 2024 income?

Social Security uses the most recent tax return it has on file, which is two years old. 2026 IRMAA uses your 2024 return; 2025 IRMAA used 2023.

Do both spouses pay IRMAA?

Yes — if you file jointly and both are enrolled in Medicare, each spouse pays the surcharge separately. Toggle “both spouses” above to see the household total.
